To facilitate the application process, please review and complete the following:

2. Master Contract for Motor Carrier Services ([an error occurred while processing this directive])
   1. Provide copy of required insurance as indicated in Exhibit A
   2. Provide copy of Hazardous Materials Registration, if applicable
   3. Provide copy of Canadian/Interstate authorities, if applicable
   4. Provide copy of blank invoice with taxpayer identification
3. Operational Practices and Guidelines 
4. Register with UIIA 
5. Review the EDI Communication Guide
