2. Supplier Questionnaire ( )
3. UPCS Carrier Agreement ([an error occurred while processing this directive])
4. Provide copy of the following items:
   1. Blank invoice with Remit Address
   2. W9 with Tax ID Number
   3. Liability of Insurance Certificate showing UPCS as Certificate Holder
   4. Hazardous Materials Registration, if applicable
   5. Canadian/Interstate authorities, if applicable
   6. DOT, MMC, or ICC authorities
   7. U.S. Customs Bond information
   8. Applicable company literature (brochures, organizational charts, service maps) and web address
